It is becoming apparent that the wisdom tooth has no place in the oral cavity of 21st century human beings and a greater numbers of people are having their wisdom teeth removed because they are not growing properly or they are impacted. The term “impacted teeth” is used to explain a tooth which has been hindered by its adjacent tooth or bone to erupt properly into normal occlusion. The inclination of the tooth determine the type of impaction and difficult of removing it. Most of the lower third molars are mesial inclined or horizontally inclined whereas most of the upper third molars are distally inclined or positioned more buccal. The explanation for the occurrence of impacted teeth in the modern generation is considered to be the gradual evolutionary reduction of mandible and maxilla taking place in modern man due to dietary change involving softer and more refined food making powerful masticatory apparatus unnecessary. In general Impacted or malposed or unerupted teeth should be removed because they can cause persistent pain, sometimes leading to infection, pathological resorption of the adjacent teeth and investing bone, on rare occasion can lead to fracture of lower. Impacted teeth can be classified as 1. Vertical impaction 2. Horizontal impaction 3. Mesial impaction 4. Distal impaction 5. Buccal impaction 6. Lingual impaction There are several techniques of removing impacted tooth and at our clinic we employ bar technique to access and remove the tooth embedded in the bone. This technique ensure minimum damage to the soft and hard tissues and had less post surgical pain and swelling for most of the cases we have attended.
